Patients can register here.
Alternitvley registration forms can be completed in person at either of our practice sites, the registration form is also on our website. For more information visit the page - Join the practice
How do I...Change my address/phone number etc? -
Patients can do this online via a form. This can also be done by emailing the practice bredburymedicalcentre@nhs.net Patients can also do this in person via reception.
Please note you may be asked for evidence of change of address.
Patients who have moved our of our practice boundary will be asked re-register with a local GP.
