Accessible Information Standard Questionnaire

It’s a legal requirement, according to the Equality Act 2010, to make all of our information accessible. We must also follow the Accessible Regulations 2018 for all of our websites, intranets, apps and online documents. All organisations that provide NHS or adult social care must follow the Accessible Information Standard by law. These standard include making sure that people get information in different formats if they need it, such as: large print, braille, easy read, email etc.

Please complete this questionnaire to let us know if you require any communication or information support needs.
