How do I...Request acesss my medical records?
Please make any requests of this nature online or ask reception to do this on your behalf.
You will be asked to complete a form via reception – this request will be reviewed by a GP and we will advise you of the outcome.
You will be able to review your record via the NHS app or Patient Access once granted.

Please complete a form requesting online access to your records – this can be done in person via reception or via email bredburymedicalcentre@nhs.net.
Your request will be reviewed by a GP – your records can then be viewed via the NHS app or Patient Access.
